Action Required: Please Review These Two Forms
We ask that you review the following forms by Friday, October 10, 2025. You can complete them electronically via our OPEROO platform, or submit a hard copy to the Attendance Office in Room 104.

Media Consent Form: This form allows you to grant or deny permission for your child to be included in school photos, videos, or communications like our school newsletter and website.

Directory Information Notice and Opt-Out Form: "Directory Information" may include your child's name and grade level for use in publications like yearbooks or graduation programs.

If you are comfortable with this information being shared, you do not need to do anything.

If you DO NOT want this information shared, you MUST complete and return the Opt-Out Form.
